HHS Region 5:
The Visiting Nurse Association of Chicago was established in 1890 by Jane Addams and other courageous and pioneering women to give underserved men, women, and children access to basic health services. The mission remains the same, and today the VNA Foundation awards grants to support home- and community- based healthcare and health services for medically-underserved residents of the metro Chicago area, including the Illinois counties of Cook, Lake, McHenry, DuPage, Kane, and Will.
The foundation provides program, operating, capital, and startup grants in support of home health care services; community and school-based services; primary care and chronic disease management; and health promotion. Priority consideration will be given to programs in which care is provided by nurses.
Grants provided by the foundation typically range from $15,000 to $80,000.
Letters of Intent for the upcoming cycle are due by March 18, 2021. Upon review, selected applicants will be asked to submit a complete proposal by April 22, 2021.
