Integration Improves the Odds: Lessons Learned

Far too often, individuals with both physical and behavioral health challenges do not get the treatment they need. In fact, according to research, primary care referrals to a mental health specialist result in a connection only about half the time. When lives are at stake, a 50/50 shot is unacceptable.

Johnson & Johnson Foundation’s Integration Project with the National Council for Behavioral Health sought to improve these odds by offering support to integration efforts at the Center for Health Care ServicesThresholds, and Zufall Health Center.

Join representatives from the National Council, Johnson & Johnson, the Center for Health Care Services, Thresholds, and Zufall Health Center for “Integration Improves the Odds: Lessons Learned”.

Attendees will learn about the innovative approaches implemented, which included:

  • mobile services
  • integrated health information exchanges
  • patient navigators

The organizations will also discuss strategies used to navigate the challenges they faced on the path towards integrated care.
